A. Electronic Forms


1. Al-Anon Group Change / Registration

This form is for changes to an existing group or to register an entirely new group. Click the button below to enter and automatically send your meeting changes to the Group Records Coordinator. (Fill out form and click Submit.)

This form is for the registration of new Alateen groups.

4. Items For Consideration

This form is to bring any concern to the attention of the Ontario South Area Chair. It can be used at any time and is not connected to any specific event (such as AWSC or Assembly).


5. Expense Claim Form

This form is to be used to submit expense claims to Ontario South. the form should be submitted no later than 60 days after the date of the expense. After you submit your expense, you will receive an email confirmation with a PDF of your submission. If you have original receipts or other documents to add to the form, please scan them and send them off by email to osatreasurer3@gmail.com with a copy of your PDF confirmation. If you want to use regular mail, please contact our treasurer for more information.

6. Donations Form

(Form 10.D. from the Area Policy Manual)

Donation and contributions to Ontario South Assembly can be made in 2 different ways:

  1. By Cheque
    Make cheque payable to “Ontario South Assembly”.
    Please use the Online Donation Form to ensure proper accounting of your donation. When you submit the online form, you receive a confirmation email with a PDF attachments of your donation details. Please print this PDF and include the print in the envelop with your cheque.

  2. E-Transfer
    Send an e-transfer to osatreasurer3@gmail.com.
    In the memo section please record your District number, Group # and Group name for proper accounting of your donation.

    There is no need for a security question or answer.

7. Upload new website event

This form is for events with date and time only. Other non-date-specific events can be sent by emailing the website coordinator directly; these will be posted on the ‘Requests for Service’ page in the ‘For Members’ section of the Ontario South website.

Alateen documents


Form 10.A. Information and permission form

Form to obtain permission for Alateen members to attend events under the care of a Sponsor/SAM


Form 10.B. Medical form

Form to obtain medical information for Alateen members and is required when Alateen members attend events under the care of a Sponsor/SAM.


Form 10.C. Supportive Al-Anon Member (SAM) Screening Application and Certification form

Form to be completed by: Member applying for certification, DR and AAPP.

Form 10.F. Assembly bid form

Form provides details of location requirements and is to be completed by the DR of the District submitting a bid to host Assembly.


Form 10.I. Executive Resume

Form to be completed and submitted by members willing to stand for Executive positions.


Form 10.J. LDC Speaker Questionnaire

Form to be completed and forwarded to Ontario South Literature Distribution Centre for members wanting to be added to the Speaker List available through LDC.
